Salary:                   £10.50 per hour

 

An opportunity has arisen for an individual to undertake the role of Senior Practitioner based in Peebles. In addition to performing all the duties of the Support Worker you will be the lead and first point of contact for the team.

This post promotes the culture of consistency and individual leadership so that the quality of support and decision making remains the same no matter who is on shift.

You should have excellent interpersonal and communication skills, accurate IT skills and the aptitude to organise and prioritise workloads. The ability to work as part of a team is fundamental to this position.

 

 

Responsibilities (include but not limited to):

 

  • Take responsibility for ensuring support plans are written for the person you are SP for.
  • Ensuring the team update support plans monthly, sharing the plans with the person or family. Making all team members and management are made aware of any changes within the plans.
  • Coach, support and mentor other team members in the best approaches when working with the person the team is supporting.
  • Assess and document risk in relevant paperwork. Keep the risk assessments up to date, share the risk assessments with the person you are SP for.  Ensure that team members and management are aware of any changes within the risk assessments.
  • Work with the person you are SP for and their team to produce their Health Action Plan and keep this up to date.
  • Produce a 3 monthly Senior Practitioner report which focuses on the person’s outcomes. Ensure this is sent to all appropriate parties (family, social worker, etc.)
  • Liaise with team members and management about any issues that need addressing with the person you are SP for.

 

 

Experience required:

 

Applicants must have experience working as Support Worker.  This is a 30 hour per week post and due to the request of the Service User, the post holder must be female.  The role requires all applicants to have a full PVG check valid for working with both children and vulnerable adults.
