BCHA is one of a group of statutory and voluntary organisations in Plymouth who have come together to form The Plymouth Alliance (TPA). It aims to improve the lives of people with complex needs by supporting the whole person to meet their aspirations, whilst also contributing towards national outcome targets in relation to statutory homelessness, children in care and care leavers, drug treatment, reoffending rates, preventing admissions to hospital and urgent care targets.

We are recruiting a Senior Practitioner, who will lead a team focusing on delivering an intensive floating service, which includes a Hospital Discharge and engagement with health and social care provision for people with complex lives.

You will act as a role model in all you do and represent the organisation and TPA professionally and positively in accordance with our behaviours and values. By putting the Alliance First Principle of Best for People Using Services and One Common Goal to support people with complex needs to feel safe, feel valued and achieve their best health at the heart of all that you do.

You will be comfortable working collaboratively across sector boundaries, with a diverse range of stakeholders.

Your strong operational, interpersonal and networking skills will inspire and motivate your team to deliver services which meet the needs of a diverse range of complex customer needs. 

DBS checks are required for roles where you will be working with or have lone contact with Vulnerable Adults and Children.
