Purpose of Role:


To support in achieving the outcomes as determined by the Social Services assessment of need.


To provide assistance with all aspects of personal, domestic and social need, both at home and within the community, as detailed in the Careplan/Support Plan produced in conjunction with Social Services.




Key Responsibilities:


  • To provide appropriate support as required to ensure that the needs of the Service User are met.
  • To provide support flexibly at variable times as agreed with the Service User.
  • To ensure that the predetermined outcomes are met within the time/budget allocated to the Service User.
  • To support the Service User to become/remain independent.
  • To work under the direction of the Service User in order to ascertain what their needs are and to listen and act upon their requests.
  • To enable the Service User to undertake any chosen activity, inside or outside of their home, by carrying out any other requests which they may reasonably make.




Measures of Success:


An annual review will take place between the Service User and Social Services in order to assess if the needs identified in the Careplan/Support Plan have been met.


The Service User will conduct a 6 month probationary review and may opt to complete annual appraisals as appropriate.









Person Specification








·       Commitment to and understanding of equal opportunities and disability awareness.

·       Flexible in hours and days that can be worked.






·       Experience of providing care or support for another individual.




·       Experience of working with disabled people of all ages.





·       NVQ Level 2 or above in Health & Social Care


·       Common Induction Standards (CIS)


·       First Aid







·       Car driver


·       Own transport


·       Must be dog friendly


·       19 hours per week, £9.00 per hour
