Extra Care Scheme Coordinator
Care Support is looking for an experienced, organised and compassionate Extra Care Scheme Coordinator to join our team.
This is a varied, hands-on role responsible for supporting the day-to-day running of an Extra Care Scheme, supervising staff and helping to ensure customers receive safe, person-centred and outcome-focused care. The successful candidate will report to the Registered Manager and will play a key role in maintaining high standards across staffing, care delivery, compliance and service operations.
About the role
As an Extra Care Scheme Coordinator, you will help oversee the smooth running of the scheme, ensuring staff understand their roles, rotas are appropriately covered and customers receive consistent, high-quality support.
Your responsibilities will include:
* Supporting the day-to-day supervision and coordination of the scheme.
* Ensuring staff understand their roles, responsibilities and attendance requirements.
* Promoting a re-abling, person-centred and outcome-focused approach to care.
* Ensuring support plans and risk assessments remain person-centred and are reviewed regularly.
* Completing and monitoring staff supervisions and spot checks.
* Coordinating rotas and supporting with shift cover where required.
* Participating in the service’s on-call arrangements, including holding a work phone.
* Providing hands-on care and support when required.
* Liaising with housing providers, customers, relatives, social workers and other external professionals.
* Maintaining accurate computerised and manual records.
* Supporting the management of complaints, incidents and other service documentation.
* Preparing reports and supporting the Residents’ Committee within the scheme.
* Monitoring staff training requirements and maintaining the training matrix.
* Supporting staff with e-learning, the Care Certificate and other training requirements.
You will also support the service with Health and Safety records, recruitment, medication processes, medication returns, MAR chart and log sheet audits, compliance spreadsheets and ensuring office records remain accurate and up to date.

You should have experience working within a care or residential setting and a good understanding of service provision within the care sector. Applicants should have, or be working towards, a relevant diploma-level qualification in Health and Social Care or equivalent and should have completed the Care Certificate.
The role requires someone who can develop positive working relationships, support effective induction and ongoing training, maintain strong assessment and review processes and work closely with residents and external agencies to help provide an integrated Extra Care service.
