Hourly Pay Rate: Negotiable, dependent on experience

Role Overview:

  • Dorset Council is seeking two qualified Social Workers to join our Home First Team, dedicated to facilitating efficient and effective hospital discharges. This is an excellent opportunity for practitioners with a passion for strength-based practice and experience in complex case management, including work related to the Court of Protection.

Key Responsibilities:

  • Support hospital discharges by coordinating with healthcare providers and community resources.
  • Conduct assessments and develop care plans that promote independence and well-being.
  • Work collaboratively with individuals, their families, and multidisciplinary teams to ensure smooth transitions from hospital to home.
  • Apply a strengths-based approach to support individuals in achieving their personal goals.
  • Navigate complex cases, including those involving the Court of Protection, ensuring legal and ethical standards are met.
  • Travel within the community to provide support and conduct assessments as needed.

Qualifications and Experience:

  • Qualified Social Worker with current registration.
  • Proven experience in the hospital discharge process.
  • Strong understanding and application of strengths-based practice.
  • Experience or knowledge of working with complex cases and the Court of Protection.
  • Excellent communication, assessment, and organizational skills.
  • Ability to work independently and as part of a team
  • A valid driver’s license and access to a vehicle for travel within the community.
